About the Opportunity
Our client is a healthcare services company in New York City that is looking for a Care Management Supervisor to join their dedicated team. This individual will oversee the day-to-day operations of the care management team, ensuring high-quality, coordinated care for patients. This role involves supervising care managers, implementing care plans, and collaborating with interdisciplinary teams to enhance patient outcomes. The ideal candidate will possess strong leadership skills, a deep understanding of care management principles, and a commitment to improving patient health and well-being.
The annual base salary range is $55,000 to $65,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Supervise and mentor a team of care managers, providing guidance and support to ensure effective patient care coordination
- Develop, implement, and monitor care plans in collaboration with healthcare providers, patients, and families
- Conduct regular team meetings to discuss patient cases, share best practices, and address any challenges
- Analyze patient data and outcomes to identify trends, opportunities for improvement, and areas requiring additional resources or interventions
- Ensure compliance with relevant regulations, policies, and best practices in care management
- Collaborate with interdisciplinary teams to facilitate seamless transitions of care and resource allocation
- Provide training and professional development opportunities for care management staff
- Serve as a point of contact for escalated patient issues and facilitate resolutions
- Maintain accurate records and documentation in accordance with organizational standards
Job Requirements
- Master’s degree in Social Services, MPA, or MPHA required
- MSW, LMSW, or LCSW in New York preferred, but not required
- Supervisory experience in Health Homes required
- Spanish-speaking preferred, but not required
