About the Opportunity
Our client is seeking a dedicated and compassionate Internal Medicine or Family Medicine Physician to join our client’s team located in Secaucus, New Jersey. This unique role involves providing comprehensive primary care services directly in patients’ homes. The physician will be accompanied by a driver and a healthcare provider to assist with patient care. This position offers a rewarding opportunity to deliver personalized care in a home setting, with no weekend visits.
The annual base salary range is $180,000 to $225,000, along with a bonus and equity ownership. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Responsibilities
- Conduct thorough assessments, diagnose, and manage acute and chronic conditions for patients in their homes
- Develop and implement individualized care plans in collaboration with the healthcare team
- Perform routine health screenings, preventive care, and wellness visits
- Educate patients and their families on health management, disease prevention, and healthy lifestyle choices
- Provide guidance on medication management and adherence to treatment plans
- Address patient concerns and answer questions regarding their health and care
- Coordinate care with other healthcare professionals and community resources as needed
- Participate in case conferences and team meetings to discuss patient care plans and progress
- Maintain accurate and detailed patient records in accordance with legal and regulatory requirements
- Ensure compliance with organizational policies and procedures
- Participate in quality improvement initiatives and clinical audits
Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Current and valid medical license in NJ
- Board certification or eligibility in Internal Medicine or Family Medicine. Experience in home-based primary care is a plus
- Excellent clinical skills in primary care and chronic disease management
