Palliative Care Nurse Practitioner

About the Opportunity

We are seeking a compassionate and skilled Palliative Care Nurse Practitioner to join our client’s multidisciplinary team in Orangeburg, New York. The team is dedicated to improving the quality of life for patients with serious illnesses. In this role, you will provide comprehensive palliative care, focusing on symptom management, psychosocial support, and patient-centered care planning. You will collaborate with physicians, nurses, social workers, and other healthcare professionals to deliver holistic care to patients and their families.

The annual base salary range is $115,000 to $150,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.

 

Job Responsibilities

  • Conduct comprehensive assessments of patients’ physical, emotional, and psychosocial needs related to their illness
  • Develop and implement individualized care plans, focusing on symptom management, pain relief, and emotional support
  • Provide patient and family education on the palliative care process, goals of care, and advance care planning
  • Collaborate with the interdisciplinary team to coordinate care and ensure seamless communication across services
  • Facilitate family meetings to discuss care goals and end-of-life planning
  • Prescribe medications and therapies in accordance with state and federal regulations
  • Support patients and families in navigating complex healthcare decisions and systems
  • Participate in palliative care program development, quality improvement initiatives, and staff education efforts

 

Job Requirements

  • Master’s or Doctorate degree in Nursing from an accredited program
  • Active Nurse Practitioner license in NY
  • Board certification in a related field (e.g., Family, Adult-Gerontology, or Acute Care NP); additional certification in Palliative and Hospice Care is highly preferred
  • Minimum of 2 years of experience as a Nurse Practitioner, preferably in palliative care, hospice, or a related field
  • Strong communication and interpersonal skills with a patient-centered approach
  • Knowledge of pain management, symptom control, and the principles of palliative care
