About the Opportunity
We are seeking a dedicated and compassionate Residential Care Coordinator to join our client’s team in Staten Island. This role is crucial in ensuring smooth transitions for residents by managing admissions, conducting assessments, and planning discharges. The ideal candidate will have a strong background in social services, excellent organizational skills, and the ability to work effectively both in the field and within their residential facilities.
The annual base salary range is $50,000 to $57,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Coordinate and manage the admissions process for new residents
- Conduct initial assessments to determine the needs and eligibility of prospective residents
- Complete necessary paperwork and ensure all documentation is accurate and up to date
- Collaborate with healthcare providers, social workers, and family members to develop individualized care plans for new residents
- Perform comprehensive assessments of residents’ physical, emotional, and social needs
- Regularly review and update care plans to reflect changes in residents’ conditions and needs
- Visit residents in the field, including hospitals, homes, and other care settings, to conduct assessments and facilitate smooth transitions
- Develop and implement discharge plans in collaboration with residents, their families, and healthcare providers
- Ensure residents have access to appropriate resources and support services upon discharge
- Coordinate follow-up care and services to ensure continuity of care after residents leave the facility
- Travel to various locations as needed to conduct assessments and support residents during transitions
- Maintain a flexible schedule to accommodate the needs of residents and their families
- Work closely with interdisciplinary teams, including healthcare professionals, social workers, and administrative staff
- Serve as a liaison between residents, families, and external service providers to ensure coordinated and comprehensive care
- Communicate effectively with all stakeholders to provide updates on residents’ status and care plans
- Maintain accurate and detailed records of all assessments, care plans, and discharge plans
- Prepare regular reports on residents’ progress and the effectiveness of care plans
- Ensure compliance with all regulatory requirements and organizational policies
Job Requirements
- Bachelor’s degree in Social Work, Nursing, or a related field required
- Minimum of 1 year of experience in a residential care setting or related field
- Strong assessment and discharge planning skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Valid driver’s license required
