Our client in New York City is seeking a compassionate and dynamic Nurse Practitioner to provide high-quality primary care services at both a school-based health site and a community health clinic. The NP will deliver comprehensive primary care to school-aged children, adolescents, and adults in a dual setting. This role includes managing acute and chronic conditions, conducting preventive health screenings, providing health education, and supporting wellness initiatives. The ideal candidate will have a passion for working with diverse populations, fostering health equity, and developing strong partnerships with school staff and community stakeholders.
The annual base salary range is $130,000 to $145,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Provide age-appropriate primary care, including health assessments, diagnosis, treatment, and management of acute and chronic illnesses
- Conduct physical exams, including sports physicals, immunizations, and wellness screenings, at the school-based site
- Offer preventive care, health education, and counseling to promote healthy lifestyles for students, families, and community members
- Collaborate with school administrators, teachers, and counselors to address student health needs and support school wellness programs
- Provide care for community health patients, including adults and families, at the community clinic site
- Develop and implement individualized care plans, including medication management and follow-up care
- Maintain accurate and timely documentation in the electronic medical record (EMR)
- Coordinate with other healthcare providers, social services, and community organizations to ensure continuity of care
- Participate in outreach programs, health fairs, and community events to promote health education and wellness
- Adhere to evidence-based practices, organizational policies, and state regulations
Job Requirements
- Current licensure as a Nurse Practitioner (NP) in New York
- National certification as a Family Nurse Practitioner (FNP) or Pediatric Nurse Practitioner (PNP)
- Minimum of 2-3 years of experience in primary care, pediatrics, or community health (preferred; new graduates with relevant clinical rotations are encouraged to apply)
- Ability to work effectively with diverse populations and demonstrate cultural competence
- Strong clinical skills, including the ability to work independently and make sound decisions
- Excellent communication, interpersonal, and organizational skills
