About the Opportunity:
We are seeking a dedicated RN for Case Management to provide comprehensive care coordination to patients. This part-time contract position requires collaboration with interdisciplinary teams to ensure efficient patient flow and optimal discharge planning. The role involves evaluating patient needs, crafting personalized care plans, and utilizing InterQual criteria to determine medical necessity.
The estimated base pay for these positions are $52.25 to $61.75 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct patient assessments and create comprehensive care plans.
- Provide discharge planning and communicate with healthcare providers.
- Collaborate with social services and other healthcare professionals to coordinate patient care.
- Monitor and document patient progress using Epic charting system.
- Stay informed on CMS guidelines and HIPAA regulations.
Job Requirements:
- BSN and active CA RN license.
- Certification in AHA BLS.
- Minimum 2 years of clinical hospital RN experience with a preference for hospital-based case management.
- Experience with discharge planning and utilization review.
- Knowledge of InterQual criteria is a plus.
