About the Opportunity:
An RN Case Manager in Santa Monica, CA, provides comprehensive case management services to patients in the inpatient general medicine unit. This full-time contract position requires expertise in clinical discharge planning, utilization management, and care coordination. The shift is 07:00-5:30 pm, Monday through Friday, with a variable day off. Business professional attire is required.
The estimated base pay for these positions is $52.25 to $61.75 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct assessments and create care plans for patients.
- Provide discharge planning and utilization management using IQ competency.
- Collaborate with interdisciplinary teams for patient care coordination.
- Monitor and document patient progress using the Epic charting system.
- Stay informed on best practices in case management and care coordination.
Job Requirements:
- BSN degree and BLS certification.
- Valid California Nursing License.
- 3-4 years preferred experience in RN case management for inpatient general medicine units.
- Experience in recent case management, utilization management, and discharge planning.
- Proficiency in utilizing InterQual Criteria and Epic charting system preferred.
