About the Opportunity:
Join the team as a Licensed Practical Nurse (LPN) at a facility in Utica, NY. In this role, provide essential care to patients in a long-term care setting, specifically focusing on MS, orthopedic, dementia, and memory care within a 40-bed unit. This is a full-time contract position on the evening shift from 2:30 PM to 10:30 PM, requiring availability every other weekend. The role includes floating to all units within the same building to ensure comprehensive patient support.
The estimated base pay for these positions are $30.86 to $36.47 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct assessments and create comprehensive care plans for patients.
- Administer medication and treatments as prescribed.
- Collaborate with healthcare professionals to coordinate and ensure continuity of care.
- Monitor and document patient progress using the EMR system, PointClick Care.
- Stay informed on current best practices in the care of MS, ortho, dementia, and memory care patients.
Job Requirements:
- Valid NY LPN/LVN license.
- Minimum of 1 year experience in long-term care settings.
- BLS certification required.
- Strong communication and organizational skills.
