About the Opportunity:
The RN Patient Care Coordinator position involves coordination of care for patients who have heart transplants. This role includes immunosuppression monitoring, prophylactic antibiotic management, monitoring for organ rejection, monitoring for infection, and all other routine patient care. This is a full-time contract position.
The estimated base pay for these positions are $60.50 to $71.50 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct specific assessments and create care plans for heart transplant patients.
- Provide management of immunosuppressive therapies and prophylactic antibiotics.
- Collaborate with healthcare professionals like MDs, APPs, and Pharm Ds to coordinate care.
- Monitor and document patient progress, specifically for organ rejection and infection.
- Communicate with multidisciplinary team members regarding patient care issues.
Job Requirements:
- CA RN License and AHA BLS certification.
- 1 year of experience in ambulatory care and experience managing heart failure or heart transplant recipients.
- Ability to float to other facilities within the healthcare system.
- Preferred experience in managing other organ transplant recipients and working knowledge of immunosuppressive therapies.
