About the Opportunity:
The RN, Transplant position involves providing comprehensive care and support to transplant patients in a fast-paced hospital environment. This role includes triaging patient phone calls, reviewing laboratory results, and supporting clinic visits in collaboration with a team of healthcare professionals. This is a full-time contract position.
The estimated base pay for these positions is $60.50 to $71.50 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conducting triage of patient phone calls and communicating patient needs to the NP/MD.
- Collaborating with the triage nurse practitioner to review laboratory results and updating care plans.
- Providing support during clinic visits, including medication reconciliation and follow-up care coordination.
- Delivering health education to patients and families.
- Handling administrative tasks such as managing faxes and processing prior authorizations.
Job Requirements:
- BLS and PALS certifications required.
- BSN preferred, with at least one year of experience in transplant settings.
- Experience with EPIC charting system preferred.
- Skills in transplant contraindications, medication adherence education, and post-transplant care.
- Understanding of post-transplant health maintenance and long-term care.
