About the Opportunity
Our client, a hospital in the Bronx, is seeking a compassionate and experienced Registered Nurse to join their weekend admissions team. This role involves evaluating patients referred for home-based hospice care, educating families about available services, and coordinating initial care plans in collaboration with clinical staff.
The annual base salary range is $90,000 to $135,100. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Conduct in-person assessments for patients referred for hospice care, determining eligibility and initiating the admissions process
- Educate patients, caregivers, and families about hospice philosophy, services, and expectations
- Collaborate with physicians and interdisciplinary team members to develop and implement individualized care plans
- Serve as a liaison between healthcare facilities and the hospice team, ensuring seamless transitions and continuity of care
- Respond to referrals from hospitals, skilled nursing facilities, and other community partners
- Maintain communication with the care team regarding patients who are hospitalized, providing updates and coordinating follow-up
- Build and maintain strong relationships with referral sources through outreach and education
Job Requirements
- Active RN license in New York
- Minimum of two years of clinical experience in an acute care environment
- Prior experience in hospice or palliative care preferred
- Graduate of an accredited nursing program; BSN is a plus
