Nurse Manager (LHCSA)

About the Opportunity

Our client, a Licensed Home Care Services Agency (LHCSA), is seeking an on-site Nurse Manager in Brooklyn to oversee clinical operations and ensure high-quality care for individuals receiving services at home. This role involves supporting field nursing staff, guiding patient care planning, and maintaining compliance across all clinical functions.

The annual base salary range is $100,000 to $125,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.

 

Job Responsibilities

  • Oversight of Clinical Staff: Provide direction and ongoing support to field RNs, LPNs, home health aides, and caregivers to ensure coordinated and effective service delivery
  • Patient Care Evaluation: Review patient assessments performed in the field and ensure care needs are accurately identified, addressed, and updated as conditions change
  • Care Plan Coordination: Participate in the development and revision of individualized care plans, ensuring alignment with physician recommendations and regulatory requirements
  • Education & Training: Offer guidance and in-service training to clinical and paraprofessional staff. Reinforce best practices in home care, safety, and patient engagement
  • Regulatory Compliance: Monitor documentation protocols and ensure adherence to agency policies as well as state and federal home care standards
  • Communication & Collaboration: Work closely with interdisciplinary team members—physicians, social workers, and community partners—to support seamless patient care
  • Quality & Performance Monitoring: Assist with audits, case reviews, and performance improvement activities to ensure consistent delivery of high-quality home care services

 

Job Requirements

  • Active Registered Nurse (RN) license in good standing
  • Previous experience in home care, community health, or a similar clinical environment is strongly preferred
  • Must have LHCSA experience
  • Strong leadership, organizational, and prioritization abilities
  • Excellent communication skills and confidence in making clinical decisions independently
  • Solid understanding of documentation standards and compliance expectations within a home care setting
