About the Opportunity
We are seeking a dedicated Family Nurse Practitioner with strong experience in community health and/or Federally Qualified Health Center (FQHC) settings to join our client’s patient-centered, mission-driven team in Brooklyn. The ideal candidate is passionate about working with diverse, underserved populations and committed to improving access to high-quality primary care.
The annual base salary range is $130,000 to $180,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Provide comprehensive primary care across the lifespan, including health assessments, diagnosis, treatment, and management of acute and chronic conditions
- Deliver culturally competent, trauma-informed, and patient-centered care tailored to community health needs
- Conduct routine screenings, health maintenance visits, preventive care, and chronic disease management
- Order, perform, and interpret diagnostic tests and labs; develop appropriate treatment plans
- Prescribe medications according to state and federal guidelines
- Provide education on disease prevention, medication adherence, lifestyle management, and self-care
- Address social determinants of health and collaborate with support staff to connect patients to community resources
- Participate in outreach efforts, health education initiatives, and quality improvement projects focused on community needs
- Communicate effectively with diverse patient populations, including those with limited English proficiency, complex medical histories, or limited access to care
- Work closely with physicians, behavioral health providers, care managers, and support staff as part of an interdisciplinary team
- Participate in case conferences, team huddles, and care coordination activities to ensure continuity and effectiveness of care
- Collaborate with external community organizations, public health entities, and social service agencies when appropriate
- Maintain thorough, accurate, and timely documentation in the electronic health record (EHR)
- Adhere to all clinic protocols, state and federal regulations, and quality and safety standards
- Participate in audits, compliance activities, and performance improvement initiatives
Job Requirements
- Current, unrestricted FNP license and certification
- Master’s or Doctorate degree in Nursing from an accredited program
- Minimum 1–2 years of relevant clinical experience in community health, primary care, or FQHC settings
- Experience working with underserved, low-income, or high-need populations
- Strong understanding of culturally competent care, trauma-informed practices, and health equity principles
