About the Opportunity
Our client in Nyack, New York is seeking a dedicated Hospice Liaison to serve as a vital bridge between hospital care teams and their hospice services, ensuring patients and families receive the support they need during end-of-life transitions. As a Hospice Liaison, you’ll work closely with hospital staff, patients, and families to identify individuals who may benefit from hospice care. You’ll play a key role in facilitating timely admissions, educating stakeholders, and ensuring a seamless transition from hospital to hospice services.
The annual base salary range is $70,000 to $95,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Collaborate with hospital case managers, discharge planners, and physicians to identify eligible patients for hospice services
- Participate in interdisciplinary rounds and care planning meetings to advocate for appropriate end-of-life care
- Conduct or coordinate hospice admissions directly from the hospital setting
- Build and maintain strong relationships with hospital personnel and referral sources to promote awareness and trust in hospice services
- Provide education to patients, families, and healthcare professionals about hospice philosophy and services
- Maintain accurate documentation and ensure compliance with regulatory and organizational standards
- Serve as a clinical resource and liaison between the hospital and hospice teams
- Review hospital census daily to identify potential hospice candidates
- Travel locally as needed; reliable transportation and valid driver’s license required
Job Requirements
- Active NYS Registered Nurse license required; BSN preferred
- Minimum of two years of recent clinical nursing experience, including at least one year in a hospital Med-Surg setting
- Prior experience in hospice, palliative care, or end-of-life services is highly desirable
- Demonstrate commitment to compassionate, patient-centered care
