About the Opportunity
We are seeking a Primary Care Nurse Practitioner to join our client’s dynamic and mission-driven primary care practice located in Manhattan. They are looking for an individual who is fluent is Spanish and English, with medical or conversational proficiency required.
The annual base salary range is $145,000 to $170,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Deliver high-quality primary and preventive care to a culturally diverse adult and family population
- Conduct comprehensive assessments, manage acute and chronic conditions, and support health promotion and disease prevention initiatives
- Collaborate closely with physicians, nurses, and allied health professionals to ensure integrated, team-based care
- Engage in care coordination, population health initiatives, and quality improvement projects that support clinical excellence and sustainability
- Participate in the development and implementation of clinical policies and best practices within the primary care team
- Contribute to the center’s patient-centered medical home model, emphasizing access, communication, and continuity of care
- Participate in peer reviews, staff training, and committee meetings to advance patient care initiatives and quality metrics
- Provide culturally sensitive care, ensuring communication and treatment approaches are accessible to Spanish-speaking patients
Job Requirements
- Master’s degree or higher from an accredited Nurse Practitioner or Family Nurse Practitioner program
- Active New York State NP license and board certification in Family, Adult, or Primary Care (FNP, ANP, AGNP, or ENP)
- Fluency in Spanish and English (medical or conversational proficiency required)
- Prior experience in family medicine, internal medicine, or community-based primary care preferred
- Strong communication, teamwork, and organizational skills with a commitment to equitable, patient-centered care
