About the Opportunity
Our client, a mission-driven healthcare organization in New Jersey, is redefining primary care for older adults with complex medical needs by bringing high-quality, relationship-based care directly into the homes. They are seeking dedicated Physicians to work closely with their clinical support team in delivering comprehensive, patient-centered care to a defined panel of patients in Camden County.
The annual base salary range is $220,000 to $250,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Deliver longitudinal primary care to patients in their homes
- Conduct in-home medical visits focused on symptom management, chronic disease care, and preventive services
- Partner with an interdisciplinary team including MAs, nurses, social workers, and other clinicians to execute individualized care plans
- Identify drivers of hospitalization risk and proactively address them through care planning
- Lead and participate in interdisciplinary care discussions and team meetings
- Coordinate specialty care, hospital transitions, post-acute services, and community resources as needed
- Complete clinical documentation accurately and efficiently using a proprietary, technology-forward platform
- Contribute to ongoing program improvement and evolving care models
Job Requirements
- MD or DO required
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Active New Jersey medical license
- Minimum 5 years of clinical experience
- Prior experience with home-based care strongly preferred
- Strong communication skills and cultural sensitivity when working with diverse populations
