Care Manager Supervisor (LMSW, LPC and LAC Required)

About the Opportunity

Our client is a leading social services non-profit looking to hire a full-time Care Manager Supervisor to provide direct supervision for a program that has far-reaching impact on the community. The right individual in this role will ensure that quality services are being provided and will uphold the agency mission. The Care Manager Supervisor is responsible for all aspects of supervision (hire, orient, train, evaluate etc.) of the Community Case Managers that report to this position.

The annual base salary range is $60,000 to $70,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.

 

Job Responsibilities

  • Be accountable for leadership, growth, profitability, and quality of services provided by Case Management Program
  • Develop and establish guidelines, standards, policies, and procedures for the program (including safety of consumers/staff) and maintain a continuing review and evaluation of the operations of the program for efficiency and effectiveness, compliance with Federal and State standards, rules, requirements and regulations and revenue goals
  • Use a person-centered approach works with consumer and other consumer identified supports to develop an individualized support plan reflecting the consumer’s personal vision for a desired life
  • Perform comprehensive assessment of consumer including health and safety, housing, vocational training/employment, physical health, income/finance, social/recreational/spiritual interests, psychological/psychiatric treatment, and other needs
  • Assessment of developmental, educational, vocational, and psychological evaluations as well as information from individuals and other service providers identified by the consumer
  • Facilitate consumer access to community resources/services, (i.e., locating recreational activities, housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health services); and assist consumer develop natural resources
  • Coordinate and monitor services, including comprehensive tracking of consumer activities in relation to care plan
  • Assist consumer and consumer’s identified support/family to develop and track implementation of plans to meet identified goals
  • Regularly meet with consumer and continuously reassesses plan to assure consumer satisfaction, the plan reflects the supports/services desired and needed and adjust action steps and timelines as needed for successful completion of the goals
  • Assess and document effectiveness and outcomes of the support plan implemented
  • Research available community resources to support plan
  • Participate as required in consumer treatment planning and multidisciplinary team meetings

 

Job Requirements

  • Must be a licensed LMSW, LPC, or LAC in the state of New Jersey
  • Must have at least 1 year of Case Management experience
  • Must have at least 1 year of leadership experience
