About the Opportunity
We are seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our client’s team in Brooklyn and provide comprehensive social work services to geriatric residents in their long-term care facility. This position is open to both experienced professionals and new graduates who are passionate about supporting the well-being and quality of life of older adults.
The annual base salary range is $60,000 to $70,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Conduct psychosocial assessments of geriatric residents upon admission and at regular intervals, identifying their strengths, preferences, needs, and psychosocial risk factors, to develop individualized care plans and interventions
- Provide supportive counseling, crisis intervention, and emotional support to residents and their families, addressing adjustment issues, coping strategies, grief and loss, and end-of-life planning, as needed
- Advocate for residents’ rights, preferences, and autonomy, ensuring their voices are heard and respected in care planning, decision-making, and conflict resolution processes.
- Collaborate with interdisciplinary team members, including physicians, nurses, therapists, and administrators, to coordinate care, address resident concerns, and facilitate communication among healthcare providers, residents, and families
- Assist residents and families in navigating the long-term care system, accessing community resources, and obtaining benefits and entitlements, such as Medicaid, Medicare, Social Security, and Veterans Affairs benefits
- Facilitate resident and family support groups, educational workshops, and psychoeducational interventions to address common issues related to aging, chronic illness, caregiving, and transitions in care
- Conduct discharge planning and transitional care coordination for residents transitioning between care settings, such as hospital to long-term care, or long-term care to home or assisted living, ensuring continuity of care and access to needed services and supports
- Document social work assessments, interventions, and outcomes accurately and comprehensively in residents’ electronic health records (EHR), adhering to regulatory standards and organizational policies
- Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to review resident cases, discuss care plans, and identify opportunities for enhancing psychosocial support and resident satisfaction
- Stay current with best practices, evidence-based interventions, and regulatory requirements in geriatric social work, participating in continuing education, professional development activities, and supervision as needed
Job Requirements
- Licensure as a Licensed Master Social Worker (LMSW) in the state of NJ
- New graduates are welcome to apply; however, prior experience or coursework in geriatrics, long-term care, or healthcare settings is preferred
- Strong interpersonal and communication skills, with the ability to establish rapport, build trust, and communicate effectively with older adults, families, and interdisciplinary team members
- Empathy, compassion, and cultural sensitivity in providing person-centered care to geriatric residents with diverse backgrounds, preferences, and care needs
- Knowledge of psychosocial assessment tools, intervention strategies, and community resources relevant to geriatric populations and long-term care settings
- Ability to work collaboratively in a multidisciplinary team environment, demonstrating flexibility, adaptability, and professionalism in addressing complex psychosocial issues and care coordination challenges
