About the Opportunity
We are seeking a detail-oriented and compassionate Intake Nurse (RN) to join our client’s Managed Long-Term Care (MLTC) team in Brooklyn, NY. The ideal candidate will have experience conducting Uniform Assessment System (UAS-NY) assessments and a strong background in long-term care. This role is essential in evaluating potential members for MLTC services, ensuring they receive the appropriate level of care.
The annual base salary range is $85,000 to $105,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Conduct UAS-NY assessments to determine eligibility for MLTC enrollment
- Evaluate patients’ medical conditions, functional status, and psychosocial needs to develop appropriate care plans
- Educate patients and families on MLTC benefits, services, and long-term care options
- Coordinate with care management teams, providers, and social workers to facilitate member enrollment and transitions of care
- Ensure compliance with state and federal MLTC regulations and maintain accurate documentation in the electronic medical record (EMR) system
- Provide ongoing clinical support and recommendations based on assessment findings
Job Requirements
- Registered Nurse (RN) license in NY (required)
- Minimum of 1+ years in long-term care, home care, or managed care
- UAS-NY certification/training and experience conducting assessments (required)
- Strong clinical assessment abilities, excellent communication, and proficiency with EMR systems and managed care guidelines
