About the Opportunity
Our client is seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join their dynamic team at a Federally Qualified Health Center (FQHC) in Brooklyn, NY. This is a full-time position serving a diverse patient population with a strong focus on community health, chronic disease management, preventive care, and health education.
The annual base salary range is $140,000 to $160,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Provide comprehensive primary care services to patients of all ages, including physical exams, chronic disease management, acute care, and preventive screenings
- Diagnose and manage common and complex medical conditions in accordance with evidence-based practice and FQHC standards
- Collaborate with a multidisciplinary team of healthcare professionals, including physicians, nurses, behavioral health specialists, and social workers
- Participate in care coordination and quality improvement initiatives
- Maintain accurate and timely electronic medical records (EMR) in compliance with organizational and regulatory requirements
- Educate patients on wellness, prevention, and treatment plans
- Support community outreach initiatives and contribute to the overall mission of improving access to high-quality healthcare
Job Requirements
- Licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of New York
- Minimum of 2 years of direct clinical experience as a provider in a primary care or community health setting
- Experience working with underserved and diverse populations preferred
- BLS certification required; additional certifications (e.g., DEA) as needed for scope of practice
- Excellent interpersonal and communication skills
