About the Opportunity
We are seeking a dedicated and compassionate Case Manager RN with specialized experience in pediatrics to manage a caseload of medically fragile children in Brooklyn, NY. In this role, you will coordinate and oversee the care plans for pediatric patients with complex medical needs, ensuring they receive the appropriate services and resources to enhance their quality of life. This position requires a deep understanding of pediatric care, strong organizational skills, and a commitment to advocating for patients and their families.
The annual base salary range is $80,000 to $110,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Manage a caseload of medically fragile children, developing, implementing, and monitoring individualized care plans
- Collaborate with healthcare providers, social workers, families, and community resources to ensure comprehensive care for each child
- Conduct regular assessments and home visits to evaluate the child’s medical condition, needs, and progress
- Coordinate services such as home healthcare, therapy, and other necessary medical interventions
- Serve as an advocate for children and their families, addressing any concerns or barriers to care
- Ensure timely documentation and compliance with all healthcare regulations and standards
- Educate families about their child’s condition, care plan, and available resources
- Participate in multidisciplinary team meetings to review and update care plans
- Provide ongoing support and follow-up to ensure optimal outcomes for each child
Job Requirements
- Active and unrestricted RN license in NY
- 3-5 years of experience in pediatric nursing required, with a preference for candidates experienced in caring for medically fragile children
- Case management experience preferred but not required
- Strong knowledge of pediatric conditions, treatments, and healthcare services
- Excellent communication, organizational, and problem-solving skills
- Ability to work collaboratively with a multidisciplinary team and manage a dynamic caseload
