About the Opportunity
We are seeking a compassionate and dedicated Case Manager with a background in social work to join our NYC client’s team. In this role, you will be responsible for performing evaluations and working with the elderly population, ensuring they receive the care and services necessary to enhance their quality of life. The ideal candidate will have a strong understanding of the unique needs and challenges faced by older adults and will be committed to improving their well-being.
The annual base salary range is $55,000 to $70,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Conduct comprehensive assessments of elderly clients to determine their needs, strengths, and goals
- Develop individualized care plans in collaboration with clients and their families to address physical, emotional, and social needs
- Connect clients with community resources, including healthcare services, housing options, transportation, and social activities
- Assist clients in navigating complex healthcare systems and advocate on their behalf to ensure access to necessary services
- Regularly assess the progress of clients and adjust care plans as needed
- Collaborate with healthcare providers and other professionals to ensure holistic care for clients
- Maintain accurate and up-to-date client records, documenting assessments, care plans, and interactions
- Prepare reports for supervisory and funding agencies as required
- Collaborate with appropriate agencies to ensure the safety and well-being of clients
Job Requirements
- Bachelor’s or Master’s degree in Social Work (BSW or MSW) from an accredited institution
- At least 3 years of experience working with elderly populations in a social work or case management capacity
- Strong knowledge of aging-related issues, healthcare systems, and community resources
- Excellent communication and interpersonal skills
- Empathy and patience when working with elderly clients
