About the Opportunity
Our client is a leading hospice care provider that is seeking a compassionate Social Worker to join its in-home hospice team in the Bronx, NY. This full-time role focuses on delivering psychosocial support, counseling, and end-of-life planning for patients and their families during one of life’s most sensitive transitions. The ideal candidate is skilled in counseling, crisis intervention, and care coordination, with experience in hospice, home care, or bereavement services.
The annual base salary range is $75,000 to $100,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Conduct psychosocial assessments and provide short-term, goal-directed therapy to patients and families
- Participate in interdisciplinary care planning and support bereavement services
- Assist with referrals, community resource coordination, and end-of-life support
- Maintain thorough and timely documentation of services provided
- Collaborate with staff, volunteers, and external partners to ensure holistic patient care
Job Requirements
- MSW from an accredited program; LMSW or LCSW eligible
- At least 1 year of experience in hospice, home care, or bereavement counseling preferred
- Valid New York State driver’s license
- Strong communication skills and a compassionate, team-oriented approach
