The organization is seeking a skilled and compassionate IMT Case Manager to join their team!
The IMT Case Manager will play a crucial role in supporting clients with severe mental illness in the community through the Integrated Mental Health Treatment (IMT) model. This position involves collaborating with a multidisciplinary team to provide comprehensive care coordination, advocacy, and support services to empower clients to achieve their recovery goals and enhance their quality of life.
The annual base salary range is $40,000 to $55,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Conduct comprehensive assessments of clients’ mental health, social, and functional needs using evidence-based tools and techniques
- Collaborate with clients, family members, and treatment team members to develop individualized care plans that address clients’ unique strengths, preferences, and goals
- Monitor and evaluate clients’ progress towards recovery goals, adjusting care plans as needed to ensure effective outcomes
- Serve as the primary point of contact for clients and their families, coordinating access to a wide range of mental health and support services, including psychiatric care, housing, employment, and social services
- Advocate for clients’ rights, preferences, and needs within the healthcare system and community settings, ensuring equitable access to resources and opportunities
- Facilitate communication and collaboration among members of the treatment team, including psychiatrists, therapists, case managers, and community providers, to ensure integrated and coordinated care delivery
- Provide crisis intervention services, including assessment, de-escalation, and safety planning, in collaboration with clients and their support networks
- Develop and implement risk management strategies to mitigate potential harm and enhance clients’ safety and well-being during periods of acute distress or instability
- Coordinate with emergency services, hospitals, and other community resources to ensure timely and appropriate responses to crisis situations
- Maintain accurate and up-to-date documentation of client assessments, care plans, progress notes, and service utilization in compliance with regulatory standards and organizational policies
- Participate in case conferences, team meetings, and clinical supervision sessions to review cases, discuss treatment strategies, and ensure adherence to best practices and quality standards
- Stay informed about current trends, developments, and best practices in mental health treatment, care coordination, and case management through ongoing training, education, and professional development activities
- Pursue licensure and certification as required by state regulations and professional standards
Job Requirements
- Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or related field
- Previous experience in case management, care coordination, or related roles within the mental health field is preferred
- Knowledge of evidence-based practices in mental health treatment, recovery-oriented care, and trauma-informed approaches
- Strong interpersonal, communication, and problem-solving skills
- Ability to work effectively as part of a multidisciplinary team and collaborate with diverse stakeholders
