About the Opportunity
Our client, a well-established primary care practice in New York City, is expanding and seeking a compassionate, patient-centered Primary Care Nurse Practitioner to join their growing team. This is a full-time opportunity that blends office-based primary care in Brooklyn with community home visits, offering variety, autonomy, and meaningful patient relationships.
The annual base salary range is $125,000 to $150,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Provide comprehensive adult primary care services in both outpatient clinic and home-based settings
- Schedule includes 2–3 days of home visits and 2–3 days in-office
- Deliver preventive care, chronic disease management, and acute care services
- Build long-term relationships with a diverse NYC patient population
Job Requirements
- Nurse Practitioner (FNP or AGNP considered; experienced providers strongly preferred)
- Commitment to excellent patient care and strong interpersonal communication skills
- Ability to work independently during home visits while remaining collaborative with the broader care team
- Spanish language skills are a plus, but not required
- We are especially interested in providers who bring warmth, professionalism, and a strong sense of responsibility to their patients
Schedule & Compensation
- Full-Time | Monday–Friday | 9 AM – 5 PM
