About the Opportunity

Our client is a Healthcare organization in New Jersey that is committed to delivering high-quality, compassionate care to their patients in the comfort of their homes. They specialize in providing comprehensive health services to the elderly, enabling them to maintain their independence and quality of life. Their dedicated team of Healthcare professionals work collaboratively to offer personalized care tailored to each individual’s needs.

They are in search of Well-Visit Nurse Practitioners in both Maplewood and Cherry Hill, New Jersey. The ideal candidate will have a strong background in geriatrics or home care, bringing expertise and empathy to the role. You will be responsible for conducting well-visits, managing chronic conditions, and collaborating with our multidisciplinary team to enhance the health and well-being of our homebound patients.

The annual base salary range is $120,000 to $155,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.

 

Responsibilities:

  • Conduct Well-Visits: Perform thorough physical examinations, health assessments, and routine screenings for elderly patients during home visits
  • Develop, implement, and evaluate individualized care plans. Monitor chronic conditions, medication regimens, and provide education on disease management
  • Work closely with physicians, nurses, social workers, and other healthcare professionals to ensure coordinated and comprehensive care.
  • Provide education to patients and their families on health maintenance, disease prevention, and managing chronic conditions
  • Coordinate and manage follow-up visits, laboratory tests, and referrals to specialists as needed
  • Identify and respond to medical emergencies during home visits, providing initial care and coordinating further treatment as necessary

 

Requirements:

  • Master’s degree in Nursing from an accredited Nurse Practitioner program
  • Current and valid Nurse Practitioner license in NJ
  • Certification in Adult-Gerontology Primary Care or Family Nurse Practitioner preferred
  • Minimum of 1 year of experience in geriatrics or home care settings
  • Compassionate approach with a focus on patient-centered care
