About the Opportunity:
Provide comprehensive social work services as a Licensed Master of Social Work in a hospital setting. Complete patient assessments, including discharge planning, while considering biopsychosocial, financial, medical, and physical factors. Respect and accommodate patients' cultural, language, religious, and spiritual values, ensuring their rights are protected. Engage patients and their families in developing and implementing care plans, coordinate with multidisciplinary teams for safe discharge plans. Educate and arrange for post-hospital care and advocate for accessing necessary services. This is a full-time contract position.
The estimated base pay for these positions are $41.25 to $48.75 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Complete patient assessments, identify needs in discharge planning.
- Include biopsychosocial, financial, medical, and physical factors in assessments.
- Engage patients and families in care plan development and implementation.
- Perform ongoing reassessment and adjust care plans accordingly.
- Coordinate with multidisciplinary teams for creating safe discharge plans.
- Educate and arrange for post-hospital care options.
- Provide short-term counseling and crisis intervention.
- Utilize community-based resources for patient needs.
- Advocate for patient access to needed services.
- Participate in continuous quality improvement activities.
Job Requirements:
- Licensed Master of Social Work in NYS.
- Minimum 2 years of experience in a hospital setting.
- Strong communication and organizational skills.
- Ability to handle crisis situations effectively.
- Proficiency in time management and prioritizing tasks.
