Our client, a prominent hospital in New York City, is seeking an experienced Licensed Master Social Worker (LMSW) for a full-time, weekday contract role. This position supports inpatient populations across medicine, surgery, and rehabilitation units and requires strong clinical judgment, collaboration, and comfort working in an acute hospital environment. The estimated base pay for this position is $60 to $65 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Job Responsibilities:
- Provide psychosocial assessments, counseling, and discharge planning for inpatient populations
- Support patients and families navigating medical, social, and emotional challenges
- Coordinate care plans with physicians, nurses, case managers, and allied health teams
- Facilitate safe and timely discharges, including referrals to community resources and post-acute services
- Document all interventions clearly and accurately in accordance with hospital policies and regulatory requirements
- Participate in interdisciplinary rounds and care conferences as needed
Job Requirements:
- Active LMSW license issued by NYSED (required)
- Minimum 1 year of hospital-based social work experience required
- Experience in inpatient medicine, surgery, and/or rehabilitation settings required
- Strong communication, documentation, and care coordination skills
- Ability to work independently in a fast-paced hospital environment
