About the Opportunity:
The Nurse Practitioner provides home-based hospice and palliative care for patients with serious or life-limiting illnesses in Escondido, CA. This is a full-time contract position. The role involves conducting assessments, developing care plans, managing symptoms, performing home visits, and facilitating goals-of-care discussions. The practitioner coordinates patient and family education and collaborates with the interdisciplinary team and community physicians. The estimated base pay for these positions are $94.05 to $111.15 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct assessments and create care plans.
- Provide patient and family education on advance care planning.
- Collaborate with interdisciplinary team and community physicians to coordinate care.
- Monitor and document patient encounters in the EMR.
- Participate in IDT meetings and quality initiatives.
Job Requirements:
- California NP & RN licenses, DEA, NPI, MSN, AANP/ANCC certification, BLS.
- 3+ years of hospice or palliative care experience.
- ACHPN certification within one year (preferred at hire).
- Valid California driver's license and insurance required.
- Spanish bilingual is a plus.
