Hybrid Intake Assessment Social Worker

About the Opportunity

A mission-driven community behavioral health program in Staten Island, is seeking a Licensed Clinical Assessment Specialist (LCSW or LMHC with Diagnostic Privileges) to join a compassionate, multidisciplinary team providing high-quality mental health and substance use services. This role is a key part of the client intake and assessment process, helping individuals access the right level of care and connecting them with critical community resources. The clinician will complete comprehensive behavioral health assessments, identify clinical needs, and guide patients toward appropriate treatment services within an integrated care environment.

Additional Compensation: Employees working on Staten Island who reside outside of Staten Island receive an additional annual commuter differential.

The annual base salary range is $75,000 to $80,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.

 

Job Responsibilities

  • Conduct comprehensive behavioral health intake evaluations, gathering psychosocial history and identifying treatment needs
  • Complete 6-8 client intake assessments daily while maintaining high-quality documentation standards
  • Evaluate risk factors such as suicidal ideation, psychosis, domestic violence, or abuse concerns, and take appropriate clinical action
  • Identify social determinants of health, including housing, financial, medical, and legal challenges impacting wellbeing
  • Collaborate with interdisciplinary care teams to determine appropriate treatment recommendations and care pathways
  • Present concise clinical summaries and treatment recommendations during case disposition meetings
  • Coordinate referrals for behavioral health treatment, substance use services, and community-based supports
  • Maintain knowledge of community resources and service networks to ensure effective care coordination

 

Job Requirements

  • Licensed Clinical Social Worker (LCSW) in New York State OR Licensed Mental Health Counselor with Diagnostic Privileges (LMHC-DP)
  • Minimum 3 years of clinical experience in an outpatient or behavioral health clinic setting
  • Strong assessment and diagnostic skills with experience conducting mental health intake evaluations
  • Ability to manage multiple assessments per day while maintaining compassionate patient engagement
  • Excellent communication, documentation, and organizational skills
  • Comfortable working with diverse and underserved populations in a community mental health setting
  • Experience with trauma-informed care and evidence-based engagement techniques
