About the Opportunity
We are seeking a compassionate and bilingual Client Care Coordinator with fluency in Spanish to join our client’s team in Brooklyn, NY. This role focuses on providing client-centered care coordination and support to individuals and families navigating mental health services. The ideal candidate will have a Master’s degree in a related field, a strong commitment to advocacy, and excellent organizational and interpersonal skills.
The annual base salary range is $50,000 to $65,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Serve as the primary point of contact for Spanish-speaking clients and families, ensuring their needs are met with cultural sensitivity and respect
- Conduct intake assessments to identify client needs, develop care plans, and connect clients with appropriate services
- Coordinate and monitor mental health and social service resources, including therapy, case management, and community referrals
- Maintain regular communication with clients, caregivers, and service providers to ensure continuity of care
- Provide crisis intervention support and escalate cases to clinical staff when necessary
- Document all client interactions, assessments, and progress in compliance with agency standards and confidentiality laws
- Advocate for clients to ensure equitable access to services, especially those with linguistic or cultural barriers
- Collaborate with interdisciplinary teams, including therapists, psychiatrists, and social workers, to provide holistic support
- Attend case reviews and team meetings to share updates on client progress and address barriers to care
- Build and maintain relationships with community organizations to enhance the network of resources available for clients
- Participate in outreach efforts to increase awareness of the agency’s services within Spanish-speaking communities
Job Requirements
- Master’s degree in Social Work (MSW), Counseling, Psychology, or a related field is required
- Fluency in Spanish (spoken and written) is required
- Previous experience in case management, care coordination, or mental health services
- Experience working with diverse populations, particularly in Spanish-speaking communities
- Strong organizational and time management skills
- Exceptional communication and interpersonal skills
- Proficiency in electronic health record (EHR) systems and Microsoft Office Suite
- Knowledge of community resources and mental health systems is preferred
