About the Opportunity:
This position offers an opportunity to provide specialized home infusion services to patients in a full-time contract role. Responsibilities include developing and maintaining referral relationships for home infusion admissions, identifying opportunities to expand services, and coordinating care in collaboration with various healthcare professionals. This role also involves assessing and evaluating patient care needs, integrating intervention skills, and educating patients and caregivers on their care plans. The position operates Monday through Friday, 9:00 AM to 5:00 PM, and requires travel to various hospital sites as needed.
The estimated base pay for these positions are $55.00 to $65.00 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct assessments and develop care plans for home infusion therapy.
- Provide education on infusion therapies such as TPN, Enteral Nutrition, and Inotropic Agents.
- Collaborate with hospital case managers, home health agencies, and physician offices to coordinate patient care.
- Monitor and document patient progress and care coordination activities.
- Stay informed on developments in the field of infusion therapy.
Job Requirements:
- Bachelor's degree in nursing from an accredited college or school of nursing.
- Licensed as a registered nurse with current registration in New York State.
- Basic Life Support (BLS) certification required.
- Infusion or home health experience preferred.
- Evidence of ability to maintain interpersonal relationships and communicate effectively.
- Ability to read, write, and communicate in English clearly and concisely.
- Physical ability to care for acutely ill patients and perform necessary clinical procedures.
