About the Opportunity:
This part-time contract position involves providing social work services to patients in an inpatient hospital setting. The role includes involvement with goals of care, end-of-life planning, and discharge planning, particularly for oncology patients. Social workers collaborate with healthcare teams to address psychosocial needs, facilitate necessary services, and support patients and families through complex medical treatments.
The estimated base pay for these positions are $49.50 to $58.50 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct psychosocial assessments and collaborate on care plans.
- Provide support for end-of-life planning and goals of care discussions.
- Collaborate with healthcare professionals to coordinate comprehensive patient care.
- Facilitate discharge planning and coordinate services to home care and necessary equipment.
- Educate patients and caregivers to facilitate discharge and care continuity.
- Document patient interactions, education, and progress according to care plans.
- Refer patients to appropriate resources and support services as needed.
Job Requirements:
- Master’s Degree in Social Work (MSW).
- Licensed Master Social Worker (LMSW) by the State of New York.
- Minimum of 1 year of experience in inpatient hospital social work.
- Experience with oncology, discharge planning, and end-of-life care conversations.
- Excellent communication skills and knowledge of psychosocial assessment.
- Experience with Epic charting system preferred.
