About the Opportunity
We are looking for a compassionate and experienced Case Manager Field Registered Nurse (RN) to join our client’s healthcare team in Ridgewood, NY. This position is a blend of field visits and office-based work, providing the opportunity to support patients in both home and community settings. The Case Manager RN will conduct in-depth assessments, develop individualized care plans, and coordinate ongoing care to ensure optimal health outcomes. The ideal candidate is an organized and proactive problem-solver, able to balance fieldwork with administrative tasks effectively.
The annual base salary range is $80,000 to $120,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Perform comprehensive assessments of patients in their homes or community settings to evaluate their health needs, environmental factors, and support systems
- Develop and implement individualized care plans that address patients’ unique needs, monitoring and adjusting as necessary to achieve optimal health outcomes
- Coordinate with multidisciplinary teams, including physicians, social workers, therapists, and family members, to deliver holistic, patient-centered care
- Conduct follow-up visits and phone check-ins to monitor patients’ progress, provide education, and adjust care plans as needed
- Facilitate transitions of care, including hospital discharges, to ensure continuity and prevent readmissions
- Document patient visits, care plans, and communications thoroughly and accurately in electronic health records (EHR), adhering to compliance and regulatory standards
- Provide health education to patients and families on managing chronic conditions, preventive care, and available community resources
- Participate in regular team meetings, case conferences, and quality improvement initiatives to support program goals and patient outcomes
Job Requirements
- Active RN license in New York
- Bachelor’s degree in Nursing (BSN) preferred
- At least 2 years of clinical experience, with a background in case management, home health, or community nursing strongly preferred
- Strong understanding of care coordination, chronic disease management, and patient education principles
- Excellent communication and interpersonal skills, with the ability to work independently and as part of a team
- Valid driver’s license and reliable vehicle for field visits
