About the Opportunity
The Outpatient Nurse Practitioner (NP) or Physician Assistant (PA) will play a key role in providing high-quality, patient-centered care to individuals seeking primary care services at our client’s outpatient clinic located on Long Island, New York. Working collaboratively with our multidisciplinary team, the NP/PA will assess, diagnose, and treat a diverse patient population, focusing on health promotion, disease prevention, and the management of acute and chronic medical conditions.
The annual base salary range is $125,000 to $135,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Responsibilities
- Conduct comprehensive health assessments, including medical history review, physical examinations, and diagnostic tests, to formulate accurate diagnoses and treatment plans
- Develop and implement evidence-based care plans in collaboration with patients, incorporating their preferences, cultural beliefs, and individual needs
- Prescribe medications, order diagnostic tests, and perform procedures within the scope of practice and in accordance with state regulations and organizational policies
- Provide patient education and counseling on preventive health measures, lifestyle modifications, medication adherence, and self-management strategies for chronic conditions
- Coordinate care with specialists, ancillary healthcare providers, and community resources to ensure continuity and effectiveness of care.
- Document patient encounters accurately and timely in the electronic health record (EHR) system, maintaining confidentiality and compliance with regulatory standards
Requirements
- Master’s degree in Nursing (NP) or Physician Assistant Studies (PA)
- Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of NY
- Minimum of 1 year of clinical experience in primary care or a related field preferred
