About the Opportunity:
This full-time contract position focuses on providing nutrition care and education to patients. Responsibilities include monitoring patients for food/drug interactions, recording interactions, and consulting with physicians for diet modifications. The role involves engaging with patients to determine nutritional needs and providing comprehensive follow-up care.
The estimated base pay for these positions are $46.75 to $55.25 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Screen and assess patients accurately and in a timely manner based on levels of care.
- Determine patients' nutritional needs and status, and provide follow-up care.
- Interview patients and/or their families to obtain history of food habits and preferences.
- Interpret pertinent laboratory, physical, and social data in the medical record as it applies to nutrition care.
- Provide nutrition education and record patient's diet and diet instructions in education section.
- Modify patient menus as needed.
Job Requirements:
- Graduate from an accredited college/university with a degree in Dietetics or Foods and Nutrition.
- Minimum one (1) year of experience in relevant settings.
- Outpatient experience and experience with dialysis patients.
- New York State Certification as a Dietitian-Nutritionist by NYSED or Registration by CDR as a Registered Dietitian.
- Preferred: Master's degree and AHA BLS certification.
