About the Opportunity:
Provide essential nursing care to patients within a skilled nursing facility setting. Collaborate alongside a medical team to enhance patient outcomes and ensure safety. Perform patient assessments, administer medications, and assist with daily living activities. Participate in patient care planning and communicate effectively with patients and families. This is a full-time contract position.
The estimated base pay for these positions are $28.60 to $33.80 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct patient assessments and participate in creating care plans.
- Administer medications and treatments as prescribed.
- Collaborate with RNs, physicians, and other healthcare professionals to coordinate patient care.
- Monitor and document patient progress and report changes to the care team.
- Stay informed on best practices in nursing care.
Job Requirements:
- Current and valid LPN license in the state of Delaware.
- Minimum of 1 year experience in a skilled nursing or similar healthcare setting.
- Strong communication and organizational skills.
