About the Opportunity:
We offer an exciting opportunity for a Licensed Practical Nurse (LPN) in Long Term Care. This role provides essential care to patients with chronic health conditions, emphasizing support for individuals with dementia. This is a full-time contract position.
The estimated base pay for these positions are $30.69 to $36.27 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct assessments and create individualized care plans for long-term care patients.
- Provide medication administration and assist with activities of daily living.
- Collaborate with healthcare professionals to ensure coordinated patient care.
- Monitor and document patient progress and changes in condition.
- Stay informed on best practices in geriatric and long-term care nursing.
Job Requirements:
- Current NY Licensed Practical Nurse (LPN) license.
- Minimum of 1 year of experience in a long-term care setting.
- Basic Life Support (BLS) certification.
- Willingness to float between units as needed.
- Strong communication and organizational skills.
