About the Opportunity:
Join our team as a Licensed Practical Nurse (LPN) in Long Term Care, where you provide essential care to patients with conditions such as dementia. This full-time contract position involves working day shifts from either 6am-6pm or 10am-10pm, including every other weekend, for a total of 36 hours a week. Responsibilities include supporting patient needs through routine care, medication administration, and monitoring their health status. Floating is required, with no exceptions. This position is an 8-week contract requiring dedication and a patient-focused approach.
The estimated base pay for these positions are $30.69 to $36.27 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct assessments to create and update care plans for patients.
- Provide essential therapies and education to patients and their families.
- Collaborate with registered nurses and physicians to coordinate comprehensive care.
- Monitor and document patient progress and report changes to the healthcare team.
- Stay informed on best practices in long-term care and dementia management.
Job Requirements:
- Current LPN license in New York.
- Minimum of 1 year experience in long-term care settings.
- BLS certification required.
- Strong communication and organizational skills.
