About the Opportunity
Our client, a respected healthcare organization in the Hudson Valley area, is seeking an experienced Registered Nurse leader to oversee Care Management operations within an acute care hospital setting. This is an exciting opportunity for a collaborative healthcare professional who is passionate about care coordination, patient advocacy, utilization management, discharge planning, and operational excellence.
The annual base salary range is $145,000 to $155,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Lead and mentor a team of Case Managers and Care Management professionals
- Oversee daily care management operations, ensuring quality, compliance, and patient-centered outcomes
- Collaborate with physicians, nursing leadership, social services, and ancillary departments to support coordinated care delivery
- Monitor length-of-stay metrics, utilization management initiatives, and discharge planning activities
- Promote timely transitions of care and reduce barriers to discharge
- Ensure compliance with regulatory, accreditation, payer, and organizational requirements
- Analyze operational and clinical data to identify opportunities for process improvement
- Support staff development, performance management, and professional growth initiatives
- Participate in strategic planning and departmental quality improvement projects
- Foster strong relationships with internal and external healthcare partners
- Assist with policy development, workflow optimization, and care management best practices
Job Requirements
- Current New York State Registered Nurse (RN) license required
- Bachelor of Science in Nursing (BSN) required
- Master’s degree in Nursing, Healthcare Administration, Public Administration, or related field preferred
- Minimum 2-3 years of hospital-based Care Management experience
- Minimum 5 years of acute care clinical experience
- Previous supervisory or leadership experience preferred
- Knowledge of utilization review, discharge planning, care coordination, and regulatory standards
- Case Management certification preferred
- Excellent communication and leadership skills
