About the Opportunity:
This position involves providing long-term care services to patients in a rehabilitation and nursing center. As an LPN/LVN, you provide essential nursing care and support to a diverse range of patients, including those with MS, orthopedics, dementia, and memory care needs. This is a full-time contract position.
The estimated base pay for these positions are $30.86 to $36.47 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct assessments and create patient care plans in collaboration with the healthcare team.
- Administer medication and treatments as prescribed by physicians.
- Collaborate with other healthcare professionals to coordinate and optimize care.
- Monitor and document patient progress effectively.
- Stay informed on the latest practices in long-term care settings.
Job Requirements:
- Active LPN NY License and other active licenses.
- 1+ years of experience in a long-term care facility.
- Current BLS certification.
- Experience with PointClick Care EMR.
