About the Opportunity:
Join our team as an LPN/LVN, providing essential care and support to patients in a long-term care (LTC) setting. This full-time contract position involves working with a 40-bed unit to ensure the health and well-being of individuals with diagnoses such as MS, orthopedic needs, dementia, and memory care challenges.
The estimated base pay for these positions are $30.86 to $36.47 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct patient assessments and develop personalized care plans.
- Provide nursing care, including medication administration and wound care management.
- Collaborate with healthcare professionals, such as RNs and physicians, to coordinate comprehensive care.
- Monitor and document patient progress, updating care plans as needed.
- Stay informed on best practices in long-term care and rehabilitation.
Job Requirements:
- Current NY LPN license verified through NURSYS and active license status.
- At least 1 year of experience in a long-term care facility.
- Current BLS certification.
- Experience with EMR systems, specifically PointClick Care.
- Strong communication and organizational skills.
