About the Opportunity:
We are seeking a dedicated RN, Patient Care Coordinator responsible for coordinating care for patients who have heart transplants. This role involves immunosuppression monitoring, prophylactic antibiotic management, and monitoring for organ rejection or infection. This is a full-time contract position.
The estimated base pay for these positions are $60.50 to $71.50 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conducting assessments and creating care plans for heart transplant patients.
- Providing education on immunosuppressive therapies and infection prevention.
- Collaborating with healthcare teams to coordinate changes in medication regimens.
- Monitoring and documenting patient progress, including lab results oversight.
- Staying informed on transplant patient care and management protocols.
Job Requirements:
- CA RN License and AHA BLS certification.
- Minimum 1 year of experience in ambulatory care.
- Cardiac nursing experience with heart failure and/or heart transplant management.
- Ability to float to other UCSF Facilities.
- Epic charting system experience preferred.
- Preferred experience in managing other organ transplant recipients.
