About the Opportunity:
The Registered Nurse, Patient Care Coordinator, provides specialized care coordination for patients with heart transplants. Responsibilities include immunosuppression monitoring, prophylactic antibiotic management, and monitoring for organ rejection and infection. This is a full-time contract position focused on comprehensive care for post-heart transplant patients.
The estimated base pay for these positions are $60.50 to $71.50 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) finalize at the time of offer.
Job Responsibilities:
- Conduct immunosuppression and prophylactic antibiotic monitoring.
- Monitor for organ rejection and infection.
- Collaborate with attending MD, APP, and Pharm D to coordinate care.
- Assess and plan patient activities with consulting departments.
- Communicate care issues with multidisciplinary team members, patients, and families.
- Oversee laboratory results for necessary immediate action.
Job Requirements:
- CA RN License.
- AHA BLS certification.
- Minimum 1 year of ambulatory care experience.
- Cardiac nursing experience with heart failure and/or heart transplant recipients.
- BSN degree required.
- Charting system experience with Epic.
- Preferred experience in managing other organ transplant recipients and immunosuppressive therapies.
