About the Opportunity
The RN Case Manager at a renowned medical center in Los Angeles provides comprehensive case management services to both adult and pediatric patients. This position requires collaboration with the interdisciplinary team to ensure patient needs are met effectively. Responsibilities include coordinating patient care from admission to discharge, utilizing InterQual Criteria, and implementing care plans while maintaining compliance with The Joint Commission and CMS guidelines. This is a full-time contract position with shifts scheduled as 4×10 hours from 07:00 to 17:30.
The estimated base pay for these positions are $52.25 to $61.75 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Conduct assessments and create comprehensive care plans for patients.
- Provide case management services and ensure care coordination.
- Collaborate with social services and interpretation services to support patient needs.
- Monitor and document patient progress, ensuring adherence to discharge planning.
- Stay informed on national safety goals and compliance requirements.
Job Requirements
- Bachelor of Science in Nursing (BSN) and Basic Life Support (BLS) certification.
- Minimum of 2 years of case management experience, with pediatric experience strongly preferred.
- Experience in utilizing Epic for charting.
- Key skills in communication, organization, and care coordination.
