About the Opportunity
Our client, a leading home health organization in Brooklyn, is currently seeking a skilled and compassionate Registered Nurse to provide in-home patient care. The individual in this role will manage all aspects of patient care, ensuring high-quality clinical services within a supportive and collaborative environment. This is a full-time, Monday-to-Friday position with competitive salary, paid medical insurance and PTO —higher pay is available for those with Certified Home Health Agency (CHHA) experience. The annual base salary range is $100,000 to $110,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Provide skilled nursing care in patients’ homes, ensuring individualized and patient-centered treatment
- Conduct assessments, develop care plans, and monitor patient progress
- Collaborate with interdisciplinary teams to coordinate and optimize patient care
- Educate patients and families on managing chronic conditions, medication adherence, and home safety
- Maintain accurate documentation and ensure compliance with home health regulations
Job Requirements
- Active Registered Nurse (RN) license in NY
- Experience in home health or a Certified Home Health Agency (CHHA) preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
