About the Opportunity:
The Licensed Social Worker in this full-time contract position provides essential support to pediatric patients in an outpatient clinical setting. This role involves conducting comprehensive biopsychosocial assessments, coordinating care, and developing customized plans of care. The position offers the opportunity to float to other facilities, enhancing diverse professional experience.
The estimated base pay for these positions are $52.25 to $61.75 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct biopsychosocial assessments and create individualized care plans.
- Provide education and case management support, including suicide/homicide/self-harm intervention and treatment options for drug/alcohol use.
- Collaborate with interdisciplinary support staff in Social Services to provide comprehensive care.
- Monitor and document patient progress using the Epic charting system.
- Stay informed on the NASW Code of Ethics and Standards of Home Health Care.
Job Requirements:
- LCSW certification.
- Minimum 2 years of experience in pediatric outpatient clinic settings.
- Experience in care coordination and case management.
- Proficiency in Epic charting system.
- Preferred: Second language proficiency, especially in Spanish.
