Case Manager Registered Nurse (Inpatient)

About the Opportunity

Our client is looking for a Case Manager Registered Nurse (RN) located in Yonkers to coordinate patient care throughout the continuum of their healthcare services. This individual will work closely with patients, families, and the interdisciplinary healthcare team to ensure high-quality, cost-effective care and optimal patient outcomes. The role involves assessing patient needs, developing care plans, facilitating discharge planning, and coordinating resources and services.

The annual base salary starts at $90,000 – $120,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.

 

Responsibilities:

  • Conduct comprehensive assessments of patients’ clinical, psychosocial, and discharge needs
  • Develop, implement, and monitor individualized care plans in collaboration with the healthcare team
  • Ensure that care plans are patient-centered and aligned with patients’ goals and preferences
  • Coordinate and facilitate the delivery of healthcare services across the continuum of care
  • Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure seamless transitions and continuity of care
  • Serve as a liaison between patients, families, and healthcare providers to enhance communication and collaboration
  • Develop and implement discharge plans to ensure safe and timely transitions from hospital to home or other care settings
  • Arrange for post-discharge services, including home health care, rehabilitation, and community resources
  • Educate patients and families about discharge instructions, medications, and follow-up care
  • Evaluate the utilization of healthcare resources to ensure cost-effective care delivery
  • Identify and address barriers to efficient care delivery and implement solutions to improve care processes
  • Monitor and report on key performance indicators related to case management and patient outcomes
  • Ensure compliance with hospital policies, procedures, and regulatory requirements related to case management
  • Maintain accurate and thorough documentation of case management activities and patient interactions
  • Provide education and support to patients and families to empower them to make informed healthcare decisions
  • Address and resolve any patient or family concerns related to care and services

 

Requirements:

  • Associate’s or Bachelor’s degree in Nursing (BSN preferred)
  • Current Registered Nurse (RN) license in the state of New York
  • Case Management Certification (CCM or ACM) preferred
  • Minimum of 1 year of nursing experience, with at least 1 year in case management or a related field
