About the Opportunity
We are seeking a dedicated and compassionate Clinical Social Services Coordinator to join our client’s Supervised Release Program team in Harlem, New York City. The Social Services Coordinator will play a vital role in facilitating the successful transition and reintegration of individuals under court-ordered supervision. This position requires a strong understanding of clinical concepts, exceptional communication skills, and a commitment to supporting individuals in navigating their path to rehabilitation and community reintegration.
The annual base salary range is $60,000 to $65,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Conduct comprehensive assessments of clients’ social, emotional, and psychological needs
- Develop individualized case plans in collaboration with clients, probation officers, and relevant stakeholders
- Monitor and evaluate clients’ progress towards their goals, adjusting case plans as necessary
- Coordinate and facilitate access to community resources, including mental health services, substance abuse treatment, housing assistance, vocational training, and educational programs
- Provide crisis intervention and support during times of heightened need or distress
- Advocate on behalf of clients to ensure their needs are met and their rights are protected within the criminal justice system and community settings
- Assist clients in understanding and navigating court orders, probation requirements, and other legal obligations
- Collaborate with legal representatives, probation officers, and other professionals involved in clients’ care to advocate for fair and equitable treatment
- Maintain regular communication with probation officers, court officials, treatment providers, and other stakeholders involved in clients’ supervision and care
- Participate in interdisciplinary team meetings to review client progress, address challenges, and coordinate service delivery
- Facilitate communication and collaboration between clients and their support networks, including family members, friends, and community organizations
- Maintain accurate and up-to-date records of client interactions, case management activities, and service referrals
- Prepare written reports and documentation for court hearings, probation reviews, and program evaluations
- Ensure compliance with programmatic and regulatory requirements related to documentation, confidentiality, and data security
Job Requirements
- Master’s degree in social work, psychology, counseling, or a related field required (MSW, MHC, MA, or LMSW preferred)

- Previous experience working in a clinical or case management role within the criminal justice system or related field preferred
- Knowledge of evidence-based practices in case management, rehabilitation, and reentry services
- Strong interpersonal skills and the ability to build rapport with individuals from diverse backgrounds
- Ability to work independently and collaboratively in a fast-paced, dynamic environment
