About the Opportunity:
As a Virtual Care Coordinator, support virtual patient care activities and promote patient-focused care. This full-time contract position involves performing various remote monitoring duties, including patient monitoring for safety and clinical needs, assisting with hospital-based telemedicine services, and monitoring hospital environmental status. Collaborate on implementing telehealth and remote patient monitoring programs, coordinate training, and participate in the development of telehealth best practices. Actively engage in clear communication with clinical teams, patients, technology partners, and vendors. Share accountability for clinical quality and safety outcomes.
The estimated base pay for these positions are $25.30 to $29.90 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct quality assurance and peer reviews, evaluate telehealth and virtual patient observation programs, and recommend improvements.
- Collaborate with hospital partners and lead case reviews, implement solutions, and monitor outcomes.
- Triage and assign cases for virtual monitoring/observation.
- Collaborate with leadership on telehealth policy and workflow development and implementation.
- Maintain operational database including relevant telehealth and remote monitoring program performance metrics.
- Identify trends and develop corrective action plans.
- Provide training on telehealth and remote monitoring workflows and platform applications.
- Serve as a liaison for telehealth platform issues and collaborate with relevant teams.
- Navigating multiple technology platforms and ensuring quality care.
- Provide continuous observation and surveillance of patients to ensure safety.
- Report variations in patient's clinical status or behavior to the clinical team.
Job Requirements:
- Minimum three years of direct patient care experience, including policy creation and quality assurance in telehealth.
- Associate or Bachelor's degree in a health-related field, preferred.
- Experience with telehealth or remote monitoring technology.
- Proficient in English (speak, read, write).
- Strong computer skills and experience with EMR systems.
- Excellent communication and organizational skills.
- Ability to multitask and attentiveness to detail.
