About the Opportunity:
This part-time contract Nurse Practitioner position focuses on providing home-based hospice and palliative care for patients dealing with serious or life-limiting illnesses. The role involves conducting assessments, developing personalized care plans, and managing symptoms to ensure comprehensive patient care. The practitioner carries out face-to-face hospice visits, engages in goals-of-care discussions, leads advance care planning, and educates both patients and families. Integration with the interdisciplinary team and collaboration with community physicians is key to coordinating effective care.
The estimated base pay for these positions is $94.05 to $111.15 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) finalize at the time of offer.
Job Responsibilities:
- Conduct hospice visits and create individualized care plans.
- Provide patient and family education on advance care planning.
- Collaborate with interdisciplinary team members and community physicians.
- Document patient encounters in the EMR system.
- Participate in IDT meetings and quality initiatives.
Job Requirements:
- California NP & RN licenses, DEA, NPI required.
- MSN degree and AANP/ANCC certification necessary.
- BLS certification and 3+ years of hospice or palliative care experience.
- ACHPN certification preferred (required within one year).
- Valid California driver's license and insurance.
- Spanish bilingual communication skills a plus.
