About the Opportunity:
This part-time contract position for a Nurse Practitioner involves providing home-based hospice and palliative care for patients with serious or life-limiting illnesses. Conduct assessments, develop care plans, and manage symptoms effectively.
The estimated base pay for these positions are $94.05 to $111.15 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct assessments and create care plans for patients.
- Provide patient and family education and advance care planning.
- Collaborate with the interdisciplinary team and community physicians to coordinate care.
- Monitor and document patient progress during home visits.
- Stay informed on best practices in hospice and palliative care.
Job Requirements:
- Possess California NP & RN licenses, DEA, NPI, MSN, AANP/ANCC certification, BLS.
- 3+ years of hospice or palliative care experience required.
- ACHPCN certification preferred at hire and required within one year.
- Valid California driver's license and insurance required.
- Spanish bilingual skills are a plus.
