About the Opportunity:
We are currently seeking a Licensed Social Worker to join our team in an outpatient setting. The role focuses on providing support and interventions for pediatric patients, utilizing specialized knowledge in child protective services and high-risk interventions. This is a full-time contract position requiring the ability to float to other facilities as needed.
The estimated base pay for these positions are $52.25 to $61.75 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct pediatric assessments and create effective care plans.
- Provide care coordination and case management.
- Collaborate with a multi-disciplinary team to ensure continuity of care.
- Monitor and document patient progress using the Epic charting system.
- Stay informed on child protective services, high-risk interventions, and cultural issues.
Job Requirements:
- LCSW certification required.
- Minimum 2 years of experience in outpatient pediatric settings.
- Bilingual in Spanish preferred.
- Proficiency in epic charting system.
- Strong skills in biopsychosocial assessment, communication, and organization.
