About the Opportunity:
The Licensed Social Worker position focuses on providing care within an outpatient setting for pediatric patients. This is a full-time contract position based at Walnut Creek/San Ramon in-person and Brentwood remotely. Duties include conducting biopsychosocial assessments, care coordination, and case management. Collaboration with a multidisciplinary team occurs regularly to ensure continuity of patient care. The estimated base pay for these positions are $52.25 to $61.75 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct biopsychosocial assessments and create care plans.
- Provide case management and care coordination services.
- Collaborate with healthcare professionals to coordinate patient care.
- Monitor and document patient progress across outpatient settings.
- Stay informed on treatment options for high-risk intervention patients.
Job Requirements:
- Licensed Clinical Social Worker (LCSW).
- Minimum of 2 years of experience in pediatric outpatient clinics.
- Experience in case management.
- Proficiency in Epic charting system.
- Preferred proficiency in Spanish as a second language.
