About the Opportunity
A large and established healthcare network dedicated to providing home care, long term care and sub-acute services is currently seeking Home Care Nurses for their Rye location. This team of interdisciplinary professionals pride themselves on providing excellent programs and services that aim to ease the burdens associated with chronic pain and they are looking for the right candidates to join their team. Part-time and full-time hours available! Valid drivers license and access to a vehicle will be required for this role.
The base pay range for this position is $45 to $52 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Job Responsibilities
- Manage and coordinates all aspects of assigned caseload
- Assess the physical and environmental conditions of the patient
- Understand, interpret, and implement reimbursement protocols
- Determine the billing source for patient care
- Consult with management on complex/unique reimbursement cases
- Provide and manage the care needs of patients
- Make/confirm referrals to other disciplines/community agencies
- Communicate and consult with patient’s MD and follow-up on problematic situations
- Document content and process of all services provided in accordance with government regulations
- Assist patients with activities of daily living
- Manage, plan, coordinate and execute the patient’s safe discharge from service
- Travel to patients’ homes/other facilities with varying environments
Job Requirements
- Registered Nurse – New York State Licensure
- Minimum of 1 year of clinical experience (Hospital, SNF, Home Care, etc.)
- Driver’s license and access to a vehicle is required
